Athletic Documents and Forms
Please submit a copy of a physician completed IHSAA physical to the Athletic Department via Final Forms. This physical must be completed after April 1, 2024 to be valid for the 2024-2025 school year. Click on the IHSAA physical form to view and then print. The student/athlete and the parent fill out the front of the form. The physician fills out and signs page 3 of the form. Please upload the complete physical form to Final Forms by following the directions on the Final Forms site.
Parents and students are responsible for submitting all forms in FinalForms for the 2024-25 school year. This includes uploading the IHSAA physical to FinalForms.
AFTER joining a team, these forms below need to be turned in to the NEMS office:
1) Middle School Student/Athletes will also be required to pay a Transportation Fee of $25 per student per athletic season. Please turn in this transportation fee form with your payment. If your student/athlete is on free/reduced lunch, this form also needs to be filled out to release your status to the athletic department.
2) A one time Parent travel permission form is required from each parent/guardian. Please send this completed form to east_athletics@nobl.k12.in.us . A transportation form and fee is also required for each sport in which a student participates.
